"Knowledge and vision" are two of the "superior fruits" (mahaphala) of the Noble Eightfold Path. They are associated with people of exceptional holiness and wisdom, Nonreturners (Anagamis) and Arahants.

Nanadassana (knowledge (nana) and vision (dassana)) is the 11th superior fruit of attainment (mahaphala).  The term 'vipassana,' which means insight is not applied here, but I believe it is understood to be a synonym.

In my experience insight (vipassana) is intuitive and revelatory, and is the product of learning to meditation deeply.  The deeper one can mediate the more intuitive, revelatory, insight one has.  Thus, those who meditate to the 2nd stage of the religious experience (2nd jhana) have fair amount of insight; however, those who consistently meditate to the depth of the 3rd stage of the religious experience (3rd jhana) have more insight.  Along with deep meditation comes liberation from addictive behavior.  One who has not fetters/addictions is one who is an arahat, etc.
